A. MasterScan FAQs
On start up, QMB MasterScan must get a license from the Licensing Server. If it does not successfully connect to the Licensing Server, the following dialog box appears:
There are a number of possible causes for this issue:
(A) Preliminary Lytec Setup
You must first set the Server Login password in Lytec. To do this, start Lytec on any machine and login to any practice. Lytec 2010

(B) ODBC Setup
QMB MasterScan uses a 32-bit ODBC connection. A 64-bit machine has 2 versions of the ODBC Data Source Administrator – a 32-bit and a 64-bit version. You must use the 32-bit version. The 32-bit version is in C:\Windows\SysWow64\odbcad32.exe. To start it click on Start\Run and enter the location as below:
At this time, QMB MasterScan requires the 32-bit version. This allows QMB MasterScan to run under older versions of Windows.
MasterScan uses the machine’s fax modem to send faxes. Please verify that the machine you’re on has one.
You can remove "stale" sessions by going to File\License Server…. You must have the System Password to logout stale sessions. Please follow the instructions at the bottom of that screen.
Windows 7 does not automatically create the folder where File DSNs have traditionally been stored. Therefore,
you have to create this folder yourself before attempting to create the ODBC connection used by MasterScan to
interface with Medisoft.
QMB MasterScan requires a TWAIN-compatible scanner. Although theoretically any such scanner should work we have found that some manufacturers have better implementations of the TWAIN standard than others. Based on our experience we recommend the following models:
In many cases you will have multiple insurance companies defined in your practice management system which in reality are one insurance company with multiple addresses. As far as MasterScan is concerned, however, all of these insurance companies can be considered as one. For example, in the case of Blue Cross Blue Shield you may have BCBS-NY, BCBS-NJ, BCBS-IL, and BCBS-CA. A BCBS EOB may come from claims sent to any and all of these companies but you would probably want to store it under a single BCBS folder within MasterScan. To do this, you must group your related insurance companies together. Here's how you set up insurance groups:
You have just enabled Insurance Groups in MasterScan. Now you have to define your groups.

B. ClaimsPrep FAQs
QMB ClaimsPrep is used to produce ANSI 837P records from Lytec. It cannot be used to convert response files into a readable format. Free software is available to do this. Here are two packages:
QMB ClaimsPrep produces professional claims (ANSI 837P). It does not produce institutional claims (ANSI 837I).
Companion guides are produced by clearinghouses and vendors and provide additional information that you need to transmit claims to them. Typically, they are found on their web sites. In some cases, however, you may need to contact the vendor and speak to a human being. Below is a list of guides that we found by searching for
You will need to find the recommended settings for the following fields:
A replacement claim is sent with Loop 2300 CLM05-3 set to 7.
Before you can send a replacement claim, you must first set the Claim Number in Lytec. To do this, open the claim in the Charges and Payments screen. Click on the Billing button at the bottom of the screen and then select Bill Options. Enter the Claim Number from the original claim in the Claim Number text box. When you run ClaimsPrep, set Submission Type to Replacement. A voided claim is sent with Loop 2300 CLM05-3 set to 8. Before you can send a voided claim, you must set first the Claim Number in Lytec. To do this, open the claim in the Charges and Payments screen. Click on the Billing button at the bottom of the screen and then select Bill Options. Enter the Claim Number from the original claim in the Claim Number text box. When you run ClaimsPrep, set Submission Type to Void.
Neither Lytec nor Medisoft has a field for Ordering Provider.
Instead, QMB ClaimsPrep uses the Outside Primary Care Provider (PCP) field.
Additionally, the "Send Ordering Provider in Loop 2420E" field must be checked for the patient's insurance company.
Caveat: The Ordering Provider is sent on the claim detail level (the CPT code level). Unfortunately, there is no way to indicate which line in a claim should or shouldn't have the Ordering Provider associated with it. QMB ClaimsPrep sends the Ordering Provider on the first detail line only. You may have to split a claim if other lines need it as well. |

C. WorkersComp FAQs
QMB WorkersComp defaults to C-4 for initial claims and C-4.2 for all others. You override this by double-clicking on a specific form in the list of claims. You will be asked to confirm that you want to switch to the alternate form.
C-4.3, C-4 Auth, MG-1, and MG-2 are available when you use the C-4/C-4.2 forms. QMB WorkersComp presents you with a list of claims to process. Place the cursor in the column labeled Form and left click. The row will now be highlighted and the cursor will be after the C-4 or C-4.2 in the row. Now, right click and you will see a drop-down menu from which you can select C-4.3, C-4 Auth, MG-1, or MG-2.
